We support many families who are experiencing difficulties, including isolation, depression, relationship breakdown, and parents or children with disabilities and special needs.
How to refer a family
Professionals and practitioners including Health Visitors, Midwives, GPs, Children’s Centres, Refuge Workers, Nurseries, Schools, Speech & Language Therapists, Voluntary organisation staff, Social Workers, can refer a family to our services. Parents and carers can also refer themselves to our service directly.
We can support a family if they meet the following criteria:
– Have at least one child under the age of five and/or are expecting a baby
– Live in Westminster, Kensington and Chelsea or Hammersmith and Fulham
– Are experiencing difficulties and wish to access befriending, therapy and/or Bump Start support
– Have given their consent to receiving Home-Start support

To make a referral, please read the Referral Procedure and Privacy Notice and complete the form below, ensuring that you complete all section.
Please note: If the requested details are not entered in full, we will be unable to process the referral and will need to contact you for further information.
What happens next
Initial visit: We aim to contact the family within two weeks of accepting a referral, to arrange a home-visit by a Family Support Coordinator. The purpose of the visit is to inform the family about our services, for the family to ask questions and to discuss their needs to see how best we can support them.
Matching: The Family Support Coordinator will discuss which volunteers are available and might be most suitable for the family at a weekly team meeting.
Match Visit: Once the most suitable volunteer is agreed, we will arrange a Match Visit to introduce the family and volunteer and agree the details of the support including how the volunteer will support the family, when the visits will take place and set review milestones.
